Frustrated AF

I’m 36 and I’ve had a red ring around my nose for years. It’s more pronounced at certain times. Sometimes my nose in general is also quite red. I was diagnosed with mostly the blush rosacea type by derm which is apparently “very difficult to treat.” I tried Rhofade cream and brimonidine gel, neither of which are particularly helpful. I use sunscreen religiously. I asked for an rx for azaleic acid which I haven’t tried yet, but I was cautioned that this is more helpful for acne-type rosacea with bumps and doesn’t usually do too much for the redness. I’m at a loss.

Challenging work situation

I work with some really difficult people at my new job (several months in now). It’s affecting patient care and my mental health. These colleagues (back office staff) act very differently when leadership isn’t around. For context, this is a new gen peds practice opening up multiple offices. The mission and vision of working with medically underserved kiddos is fantastic. Leadership frequently talks about building a positive work culture, liking where you work, and being open to feedback from the ground level.

I’m considering approaching leadership about the tension in the office because they have zero idea what’s going on. I’m conflicted. I don’t think I would lose my job, but I also don’t want to stir the pot. Anyone been in a similar situation?

Faster IPL?

Is there a faster IPL out there than the one I’m using? The whole process takes an incredibly long time and I find it hard to commit to that, even if effective. I’ve been using the JOVS X 3-in-1 IPL Hair Removal Device since 2024 (previously used Braun).

New compounding pharmacy partner

It looks like my telehealth provider changed their compounding pharmacy partner, probably because BPI stopped production. My previous tirz vial was manufactured by BPI (503B) and dispensed by The Pharmacy Hub. This time the vial was compounded and dispensed by Pharmara Labs d/b/a the Formulation Compounding Center (503A), which is based in Texas. Both are compounded with B6. I looked up the Formulation Compounding Center on the Texas State Board of Pharmacy website, and it looks like they have a clean record. Anyone have any experience with them?

Swelling

I originally posted photos but deleted my post because I got worried about my photos being online.

Anyway, I’m 25 days post op and I’m kind of freaking out. I underwent an open rhino with lateral osteotomies. My surgeon said to expect to be 80% healed at 1 month, but my nose is still really swollen…that or it’s just big. It’s gradually improving but the size is not at all what I envisioned my nose to look like from left to right. I already took a 6 day steroid course. My post-op appt is next Thursday, hoping for steroid shots then.

I underwent a rhinoplasty on 4/13 where my bridge was filed down and repositioned, bone cuts were made on both sides to narrow the bone width, and my tip cartilage was trimmed/reshaped/supported with cartilage grafts.

I’ve been taping day/night and completed 6 days of oral steroids on 5/3.

This is my progression thus far. I’m trying to be patient but it’s hard when my nose is still so big. I’m struggling, you guys. I am just so curious about the final outcome! I’m posting to see if anyone else relates or had a similar experience. This has been such a journey!

Trying to draft this post while being mindful of the rules.

I was prescribed 1 mg tirz through Maximus Tribe. It arrived in their branded “microdose” vial. I am still in the process of titrating (currently at 1.75 mg, may go up more, I don’t know). My doctor said it was cool to titrate, but we only get one consult visit.

I’d like a vial that will allow me to titrate freely but stay within the microdose range of <2.5 mg.

Normal BMI. Prediabetes range, FMH diabetes.

Prefer telehealth over gray. Gray scares me.

Thoughts? I just don’t know where to begin.

I always have a red ring around my nose. This has been the case for years. I also often have a red nose in general. My derm diagnosed me with rosacea and prescribed Rhofade cream. I don’t feel it has been particularly effective. Thoughts?

I skipped one shot week of compounded tirz April 9th for my surgery and since then, I don’t feel like it is working like it was for quieting my food noise.

While I usually take 1.5 mg every Thursday, I mixed things up the last week of April since I felt like it had worn off. I took 0.25 mg on Tuesday, April 28th and 1.25 mg on Thursday, April 30th. It’s hard to say if this really did much.

I’m thinking about increasing to 1.75 mg total. But I feel like it’s worn off in general, so I don’t want to wait until Thursday. I’d like to do 0.25 mg today (Monday, May 3rd) and my usual 1.5 mg on my shot day Thursday (Thurs, May 7th). Or should I do 0.5 mg today and 1.25 mg Thursday?

What do you guys think?

My individual NSO occurrence policy renewal is $4500 in FL! Context: PNP at pediatrician’s office.

I am a covered provider under my employer’s professional liability policy. Per my contract, my employer will pay for tail if I’m terminated without cause or if I resign with proper notice.

I’ve always had my own individual policy in addition to my employer’s in CA (where I used to practice), but now I am questioning if it’s necessary. It’s so wildly expensive in FL.

Thoughts?
