If you've been on compounded semaglutide or tirzepatide, you've probably seen the news. Multiple 503(b) outsourcing facilities are ceasing production or being shut down by the FDA. Patients are losing access to medications that were working for them, and a lot of people are panicking.
I run a telemedicine practice and treat thousands of GLP-1 patients. Here's what I'm telling mine:
**What's actually happening:** The FDA is tightening enforcement on 503(b) compounding facilities. Some are shutting down voluntarily, others are being forced out. This does NOT mean all compounded GLP-1s are going away — but it does mean the landscape is shifting fast.
**What this means for you:** If your pharmacy closes, your prescription doesn't transfer automatically. You need a provider who has relationships with pharmacies that are still operating, compliant, and shipping legitimate product.
**What to look for in a new provider or pharmacy:**
* Are they using a licensed, FDA-registered 503(a) or 503(b) pharmacy?
* Is the product third-party tested?
* Is a real physician reviewing your case, or is it a prescription mill?
* Can they ship to your state?
**What to watch out for:** The 503(b) shutdowns are pushing patients toward subscription telehealth platforms and DTC weight loss companies. Some of these are backed by venture capital, not physicians. Before you sign up, ask yourself:
Are you seeing the same doctor each time, or a random provider from a rotating network? Is the medication coming from a vetted pharmacy, or are they white-labeling product and marking it up 3-4x? Are you locked into a monthly subscription you can't cancel easily? Do they actually adjust your dose based on your response, or is everyone on the same cookie-cutter protocol?
A lot of these platforms spent more money on Instagram ads than on clinical infrastructure. They're designed to acquire patients fast, not treat them well. When the compounding landscape shifts again — and it will — those companies pivot or shut down. Your provider shouldn't be a startup that might not exist next year.

I have several international trips coming up this year ranging from 7-15 days. Each vial will last me 5 weeks, so I will need to travel with 1-2 vials on each trip. I’d love to find a small battery powered or rechargeable travel case, but everything I’ve found on amazon either requires freezer bags or holds more medication than needed and is therefore quite expensive.
I will NOT have access to private refrigerators for a few of these trips (including a glamping safari) which is why I’m prioritizing something that requires batteries or a recharge (I will have electricity).
Anyone have a travel case that fits this request and that you’d recommend? TY!
I cant seem to find any posts related to the oral Sermorelin, so I figured I’d post about my experience. Ive used the sub-q Sermorelin before but was interested in trying a different delivery system after some injection site reactions. Saw ODT version (oral disintegrating tablets) offered on the Gimme website, and I decided to place my order on April 18, was prescribed on April 19, then sent to pharmacy and shipping label was created, then….drum roll: I never received the product and am out $129.
I’ve tried using the website to contact customer support, but its ability to “connect” is a hit or miss (seriously, the chat disconnects more often than not). I then tried reaching out on Reddit to Chloe, who tried her best but is probably getting slammed with all the recent tirz orders, so no movement there. Then I tried email, but after a few exchanges with no progress, I finally gave up and requested a refund, but was told I can’t be refunded because the “product was delivered”… WAIT WHAT!!?? The product never even shipped, it’s been three weeks, just give me my money back!
On compound tirz for 5 months. Usually my vials come with a 28 day BUD. Last shipment had 21 days marked which doesn't give me enough margin if I'm rationing for a week.
Is this a quality issue? A stability issue? A this batch is closer to expiring than usual situation? I don't know who to ask. My pharmacy support said that's the BUD assigned by the compounding pharmacy which is true but doesn't actually answer my question. Also unrelated but my air fryer broke and I'm grieving.

I was prescribed 1 mg tirz through Maximus Tribe. It arrived in their branded “microdose” vial. I am still in the process of titrating (currently at 1.75 mg, may go up more, I don’t know). My doctor said it was cool to titrate, but we only get one consult visit.
I’d like a vial that will allow me to titrate freely but stay within the microdose range of <2.5 mg.
Normal BMI. Prediabetes range, FMH diabetes.
Prefer telehealth over gray. Gray scares me.
Thoughts? I just don’t know where to begin.
Genuinely curious how other people have structured this because mine feels kind of chaotic still. I'm 4 months in and I still don't have a clean process. I do it at different times, different spots, sometimes forget to let the pen warm up, once did it standing in a parking lot because I realized I'd forgotten.
Is there a routine that actually works or is everyone just kind of winging it?
Hi everyone, I’ve been on tirzepatide for the past 3 months. Around 3 weeks ago I changed my dosage from 2.5 to 5mg. Since then, 3-4 days a week after taking my shot I feel nauseous, bloated, indigestion, and fatigue. Could this be a sign to lower my dosage? Has anyone had experience with this before? I had little to no symptoms before increasing my dosage.
