Rotten smell coming from inside nose for 1+ week + chronic mucus/congestion + impacted wisdom tooth. Sinus issue or dental infection?

20M, non-smoker, non-drinker, no tobacco/gutkha/paan use.

For the past 1+ week I’ve had a very rotten/pungent smell coming from inside my nose. I notice it especially when I wiggle/move my nose around. Sometimes the drainage going from my nose into my throat also smells/tastes foul.

I also frequently get blocked nose/runny nose and have had congestion issues for around the past 6 months.

Another thing I’ve noticed is that whenever I catch a cold, I end up coughing and spitting out a LOT of mucus/phlegm even after the cold starts improving. These mucus coughing episodes usually last at least 2 weeks and sometimes close to a month before I fully recover. During those periods I cough a lot throughout the day and spit out a large amount of mucus repeatedly.

On 14 April 2026, during one of these mucus episodes, I noticed a very tiny amount of blood mixed with the mucus/phlegm (small streaks/spots only, not a large amount). I’ve attached a picture of that as well.

Important dental history:

  • Lower left impacted wisdom tooth diagnosed in May 2024
  • Dentist advised removal and warned it could become painful/infected later if left untreated
  • I did not get it removed
  • Food frequently gets stuck around that tooth under the gum flap
  • I also have fillings that were done many years ago
  • My oral hygiene has honestly not been great and I often skip brushing, especially at night

Current symptoms:

  • Sensitivity to sweets and cold
  • No facial swelling
  • No fever
  • No severe headache
  • No difficulty opening mouth
  • No pain while chewing
  • No visible gum swelling
  • No bleeding gums
  • No smoking/vaping/alcohol

The smell feels more like it’s coming from the nose/sinus area rather than directly from the mouth.

I only have this cropped X-ray image from 28 May 2024 attached. Could this impacted wisdom tooth or possible dental infection cause sinus/nasal odor like this, or does this sound more like chronic sinusitis/post-nasal drip?

Should I see a dentist first or ENT first?
