Popping and Cracking

Hello everyone!

I am a little over four months out from a bimalleolar ankle fracture with dislocation and ligament damage.

I had ORIF with systematic screws

I have been weight bearing since 11 weeks (March 3rd)

Things have been going well overall. I can walk without a limp 98% of the time, though there is a slight one when I first get up before I’ve stretched etc. I can do stairs, but only one step at a time. I can go up steps without holding on, but I still have to hold on to go downstairs

Swelling has improved, but still gets pretty ridiculous by the end of the night, but it doesn’t hurt other than some tightness/discomfort most of the time

Tonight though, I was walking and I heard a pop more towards the middle of my outer calf up towards the top part of my scar. It didn’t hurt, I don’t think, maybe a one or a two, but I’m still able to walk normally and I’ve made sure to focus on my gait

Since then I’ve had lots of pops and crackles (about 10-20 throughout the night) in various spots, some around the ankle and ankle hinge, a couple around the calf where I heard the pop, just kind of everywhere

Then at one point it seemed like where I heard the pop was shifting… but there’s no pain, other than some tightness. Maybe when I first stand up there’s a bit more now in that lower calf than there was previously, but after a few steps it’s fine. It could also be I’m just hyper aware since I heard the poop also

What do you all think it is? Is be much more concerned of I was in pain or not able to walk… but maybe I should be concerned?

6 months post ORIF

Ran 6 miles today, 6 months after my ORIF + syndesmosis repair for a trimal ankle fracture. Posting this because when I first broke my ankle, I had so much fear that I would never run again, or that I would lose all of my fitness/strength. My biggest recommendation is to GO TO PHYSICAL THERAPY as soon as you are able!! I started PT before I was even weight bearing to work on mobility. I couldn’t even move my ankle the first time I tried. It truly does get better yall. ❤️

Remove hardware or Not?

I had Orif Sx on 2/11 on my ankle. I’m seeing a lot of posts about people having their hardware removed. Prior to surgery, my surgeon told me that the hardware would be there to stay. After seeing so many people having their hardware removed, now I’m questioning why I was told the hardware stays put. Was anyone given the option? Was anyone else told the hardware has to remain in?

I’m building a scar massager that combines heat AND vibration — would anyone actually use this?

Hey everyone, I’m an engineering grad working on a handheld device to help improve scar healing (flexibility, appearance, and comfort).

Most scar massagers I’ve seen (like Norco, Rolyan, and Point Relief) only use vibration. I’m exploring adding controlled heat as well, since warmth can help increase blood flow, relax scar tissue, and make massage more effective.

The device has adjustable vibration strength (25% to 100%) and heat (30°C / 86°F to 45°C / 113°F). You can use heat and vibration independently, or both together.

Would love some honest input:

  • Have you used a scar massager before? Did it help?
  • Do you think adding heat would make a difference?

Not selling anything, just trying to see if combining heat and vibration actually solves a real problem.

Early sign of dependency?

Broke humerus 14d ago, prescribed 5 mg vicodin, surgery 8d ago. Took vicodin and gabapentin round the clock first couple of days and then vicodin only at night days 4 - 6 after surgery.

Day 6 (12 after break) I felt no pain during day so went without at night. Slept terribly, in pain all of day 7.

Day 7 (13 after break), took 5 mg before going to sleep. Slept like a rock and zero pain all of day 8.

Day 8 (14 after break), went without again, slept terribly, now awake and in pain.

Is this just normal or rebound pain/early opioid dependency? If so how long does it last? Can I take tylenol pm or something to sleep in the meantime?

Ankle ORIF - 32 weeks pregnant

This forum has been super helpful to me over the last few weeks so I wanted to share my own experiences in case they help anyone else along the way!

On 4/12 I tripped on the last step of our patio at 30 weeks pregnant and pretty well flipped my foot backwards (oops). An ambulance ride and an initial closed reduction later, I was in a splint for a week and a half waiting for ORIF surgery for a trimalleolar fracture of my ankle. Thankfully, I instinctively prioritized protecting my big preggo belly when I fell so the baby was completely unaffected. We had done a few labor classes ahead of my fall so when in the ER I knew I could accept medications like fentanyl which helped a lot with the initial closed reduction.

After consulting with maternal fetal medicine and OB anesthesia, alongside my Ortho team, we elected to proceed with full general anesthesia for the operation. I also consulted a trusted friend who is an OB who advised on the VERY minimal risks of general anesthesia at this state in pregnancy. So to any other pregnant women suffering a traumatic orthopedic injury, hopefully this is reassuring to you. The OB team monitored fetal heart rate through the entire operation on 4/22. A nerve block was also administered to my affected leg and OB opted to put me on a magnesium drip early on in the operation as well. Surgery was successful and I had 2 plates and a total of 13 screws installed. Baby was a champion and according to the OB nurse was visibly dancing around in my belly during most of the operation so that's fun. The surgical team did elevate me via one glute prior to putting me under so I was not fully prone during the operation.

For pain management following the operation, I am allowed to utilize oxycodone and Tylenol. My OB also preemptively put me on Lovenox to minimize any risk of DVT so if you're undergoing a similar operation, worth discussing this with your OB. The first night home was fine but the second night the nerve block wore off fully and I was in intense pain (far more than the pain of the initial break). This subreddit was really helpful in informing me how rough coming off the nerve block could be, so I want to echo those comments but also say "it gets better". My discharge papers gave me a number for the on call surgical resident who I called at 10 pm the night of my extreme pain and it was nice to just have some reassurance on things going to plan. I told him I'd been wiggling my toes every so often to keep blood moving and he advised me to stop that until the nerve block pain had dispersed. He said to focus on elevation and rest and I could worry about toe wiggling in a few days once the pain has subsided. That helped a lot because the next day I more or less laid perfectly still with my foot as high as I could get it for the majority of the day and the pain all but went away. So I had about 20 hours of ick and now I'm definitely on the other side. If you're going through nerve block hell, it will end. 3 days post op and I'm down to only taking Tylenol for pain.

Through all of this, I've learned a fun statistic that 8 % of pregnant women experience some sort of traumatic Ortho event - so if you find yourself in this boat as well, you can survive this!!! Not a fun club to join, but go easy on yourself and allow yourself to rest. My friends keep reminding me that even though I'd rather be in full nesting mode right now, all baby really needs when they come home is the bassinet, a few diapers, and to be fed. So take care of yourself and heal so you can take care of baby when they arrive!

Lateral Malleolus ORIF - Expectations/Fears vs Reality: 9 weeks in — Some hope for those in recovery

broke my ankle ice skating early this year and honestly Reddit helped me a lot when I was panicking BUT also made me panic a lot, so here’s my timeline / some niche advice for anyone going through the same thing.

Everyones experience / progress is different - i had one of the “simplest” fractures possible - only one bone and it’s the fibula

stats: 21/22 year old female, bmi and body fat in healthy range (underrated factor in your healing process)

ORIF surgery (plate + screws + lag screw fixing distal fibula)

• First 2/3 weeks – plaster/cast, non-weight bearing, leg up ALL DAY! had HORRIBLE nerve / zap sensations this which i thought would last forever (saw so many scary reddit posts)

• cast removed after 2w6d, moved to boot, told partial weight bearing (but didn’t since i was scared). nerve pain almost immediately dissipated and completely stopped after 24h - my nerve pain was entirely from the pressure of the cast

FAW

• Around 4 weeks post-op – surgeon said repair looked excellent, alignment was good, scar healing well, and I needed proper physio + to start progressing to full weight bearing in boot ASAP - for simpler injuries, there r 0 concerns about any sort of further damage / risk of early weight bearing at all

  • i don’t remember the exact timeline but from 4.5 weeks post-op to 9 weeks post-op, i progressed from 2 crutches in a CAM /air boot to no crutches FWB
  • i started pushing myself by standing up barefoot and shifting weight side to side from week 5.5 which i think really helped me progress quicker
  • my physio took me immediately from boot with crutches to barefoot - NOT sneakers with crutches - was painful and scary at first but i think it forced by foot to wake up again much quicker - lots of reddit threads scared me about this but it’s okay to go barefoot early!
  • i got a second opinion, and she said sleeping with the boot was not necessary. pls ask your surgeon about this specifically - it depends on how plantar flexed your foot is after surgery. 
  • for 8 weeks after surgery, i had to sleep with a special heel cushion because the pressure on my heel was very uncomfortable at night  - aside from that sleeping was fine 
  • i began to sleep on my side at 7 weeks (not the side of the fracture) - i think i could’ve started earlier, but pls err on the side of caution
  • within week 9 alone, i went from still heavily needing one crutch to being able to take stairs without crutches, turn with no issues, and begin to function as normal. things happen MUCH quicker than you may think - lot of reddit threads scared me about my ability to take the stairs being hampered for months, but it was solved within 3 days of practice 
  • another thing i saw a lot that worried me but never happened is struggling to put on shoes or socks - the first 3-5 days in the boot socks were a major struggle to put on, and normal ankle socks caused serious swelling. the main solution to this is a) using tubular bandages - the ones for knees so it’s looser and b) being patient. after max 5 days of doing this, socks became a non-issue
  • similar to above, i heard that with a lateral malleolus fracture, shoes can be uncomfortable after - but so far i have experienced minimal discomfort wearing my running shoes - but i can’t say it’s 0. prob 2-3/10 discomfort MAX but usually 1/10
  • my foot clicked every time i stepped when i started FWB - it solved itself over time and was nothing to worry about at all
  • i still limp, yet to understand the cause of this 
  • running: i’m cleared to run at 12 weeks, but i have no plans to - you don’t always have to wait 6 months
  • appointments - i see a lot of people on reddit have appointments at 6 weeks, 3 months, 6 months, 1 year - my doctors all said that as of now, 2.5 months after, they don’t see any need for me to have to follow up about this injury again unless i seriously mess things up - i would like to but i probably won’t 
  • by week 9.5, swelling is already very minimal, especially in the mornings. most of my foot looks identical now apart from around my ligaments and the ankle joint 
  • weight gain / muscle loss: my weight has stayed almost constant, i think it changed by 0.5kg overall - BUT my left leg is much softer than my right, so overall my body fat % has definitely gone up. 
  • diet and regimen: i avoided sugar during this time since it causes inflammation (but i still had donuts and cookies from time to time) - also ate extremely high protein - casein and whey multiple times a day, low fat ground beef 3x a day etc. took collagen powder (applied nutrition), vitamin d 2000 iu, calcium 1000mg, iron etc - get good quality supplements that are formulated well
  • exercise - i was told to start cycling on a stationary bike from week 5.5 by the physio. and by week 8, i was doing home workouts on youtube. 
  • my surgeon told me that i can use the smith machine at the gym from now - static load doesn’t matter at all

What helped:

• Physio once a week - really good for pushing yourself beyond what you think is possible. But most progress does happen at home - i don’t think twice is needed

• Daily ankle mobility (especially dorsiflexion + side to side)

• Scar massage - again, be careful with timing but this helps soften the tissue - at first i was so scared because i didn’t want to dislodge the metal, or even feel it

Early FWB Days: better to be in shoes or bare/sock feet?

Hey all,

I'm just two days FWB and I don't see my physio until next week.... just wondering if others have found it more comfortable/beneficial to walk around house in shoes or in bare/sock feet? I am in Canada and we don't wear shoes inside the house, so I've just been puttering around home in my sock feet. I am home all day with my babies while on mat leave, so no reason to wear shoes. I am pretty sore, though, even in compression socks. Wondering if shoes would make it better? Appreciative of any shared experiences! Thanks!

Swollen elbow 5,5 weeks after surgery - how to get rid of the swelling

Hey everyone,

In February I broke my elbow radius head. It took some time before the first surgery (15 days later) where I received 3 titanium screws.

A week later I had to have a second surgery because of a reflex and the doctors thought one of the screws I had moved (they couldn't recognize it on the CT, hence the 2nd surgery). It turns out all screws were in place so they cleaned up some tissue and some hemarthrosis and adhesions.

So now I am 5,5 weeks after the second surgery and below the elbow I still have swelling. I started lymph drainage this week, but it is only once per week, so no idea if it will help.

I have 3 times per week PT and do everyday exercises at least 3-4 times.

Yet the swelling is there and hinders my recovery.

Do you know anything that can help with that? I also have a compression sleeve since one week and wear it almost all the time.

Update: Trimalleolar fracture Weight Bearing in under 3 weeks post op

Update! 🩷💪🩼

March 31: Fell into a trench → left ankle fracture-dislocation

April 2: Surgery (ORIF) with plate + screws + syndesmotic screws

April 2–21: Non-weight-bearing in splint, focused on elevation and healing

April 21: Sutures removed → placed in walking cast, cleared to bear weight

Now: Gradually increasing weight-bearing and walking (mostly independent at home)

May 19 (expected): Cast comes off → transition to boot + start rehab/mobility work

Early June: Walking independently (projected)

Late July/August: Return to light hiking/activity (projected)

At the time of the 🦿splint and suture removal things looked in good shape with minimal swelling. 🩷🩷🩷

I'm executing a 10 min every hour walking plan with increased weight bearing.

The cycle:

5–10 min → walking / standing / tasks

30–45 min → elevated above heart

10–20 min → neutral sitting (foot supported)

👉 Then repeat

Days 1–3:

🩼🩼🩼 25–40%

Light pressure

Use both crutches

Days 4–7:

🩼🩼 40–60%

More confident loading

Less leaning on crutches

End of week:

🩼 moving toward 60–75%

I need to see the plan. 🤓

At the end of this cycle I'll evaluate my progress, reassess and write the next 7 day plan.

My mind will allow me to accomplish anything as long as I can imagine it in very small achievable chunks.

I need it. 🩷

Post-Trimal shoe?

I will be 12 weeks post-ORIF for Trimal on Saturday. I have been cleared to walk in a “supportive shoe” half-day starting next week. I typically wear Saucony and my doc said they’d be fine. She just said no Converse or Crocs.

Any ideas for a supportive shoe? I work in an office, but can wear sneakers — they should just be pretty plain.

Arm just hurts always

Operated Thursday for humerus spiral fracture and radial nerve decompression. Trying to take vicodin only at night and.tylenol during day since it's been 1.5 weeks from initial fracture but pain is all I can think about. I have gotten nothing done at work and have to go back in person next werk. I am only at a 3 out if 10 but don't know how I can do this. What if it hurts forever? What if I lose my job?
