When I play against normal champs I perform well, but against some off meta or ranged BS I get stomped, what's the problem?

Latest example: heimer, leona(pta Q max crit leona?!),elise(DnD?!)Malhazar(?), but against normal tops I perform well, what's my(most probable) problem?
