Will a sliding genio of 4mm change my front?

Hi, I have scheduled a genioplasty with 4mm of advance only horizontal in two weeks (3mm retrognathia), but I'm very worried that it will change for bad how I look from the front or that it will become more elongated by the front.

My Dr. assured me that it will not look longer

Can laser co2 skin-wrap the skin of my nose?

I did a rhinoplasty 1 year and a half ago, my thick skin doesn’t stick to my new structure and my body fills with edema the void (with time that becomes scar tissue)

I did try to put tape for 12h and afterwards my nose was so snatched! It’s edema.

I tried Kenalog injections twice 0.8 conservative doses, but the result it’s only temporary.

As many time already passed I believe it’s impossible for my skin to stick to the new structure and I might need a revision but first I’d like to try other things

Any recommendations? Would you try Kenalog again knowing this? Would laser co2/microneedling work?

My facial ultrasound after 5 years of "dissolvable" fillers. Think twice.

I just got a facial ultrasound and the results are a massive wake-up call for anyone who thinks dissolving is a simple undo button. 

Few months ago, I decided I wanted my old face back, so I dissolved the filler in my chin. I expected a reset. Instead, my face now feels off and my chin has a strange shape that isn't mine.

  1. Your tissue might NEVER be the same

My ultrasound report shows that the process of disolving (and the material) caused a marked tissue reaction and a physical thickening of the planes in my chin. 

• It’s not just leftover gel making my chin look different; it’s a marked alteration of the hypodermic tissue.

• My internal anatomy has been structurally modified. The report calls it a modification of the underlying histomorphological structures.

• Essentially, the inflammation and the thickening are now part of my face’s new architecture. My tissue fough the filler and the dissolving process, and it changed the shape of my face in the process. 

2. The Integration Trap: Why I can't dissolve the rest

I asked my doctor to just clean up the rest of the HA in my face. The ultrasound revealed why that is a dangerous idea. In several areas, the filler is "No Detected," which the report explains as a perfect integration of the implant in the tissue.

• The filler is no longer in "pockets" or "blobs." It is woven into my own fibers and cells. It is indistinguishable from my natural anatomy. 

• Because it’s so integrated, using more hyaluronidase is a huge risk.

• If we try to dissolve "integrated" filler, we risk dissolving my own healthy tissue because the enzyme can't tell the difference anymore. 

3. Temporary is a lie

The ultrasound found filler in my nasolabial folds that was injected over 5 years ago. It’s still there, visible, and detected.

2. The "Fat Creation" Trap

My doctor explained something interesting. For HA to give volume, it uses cross-linking agents, molecules that join the acid together so it stays stable and doesn't dissolve immediately. He told me that these molecules, and the way they interact with our tissue, can actually trigger the creation of more fat in the face. You aren't just adding temporary gel; you might be permanently increasing the fat volume in your face due to this reaction.

My advice? If you think you can just hit the reset button if you don't like your filler, think again. My hypodermis is now in an "altered state" with a modified structure. Once you start the cycle of filling and dissolving, you aren't just moving gel around—you are potentially changing your biological tissue structure forever
