Hoping someone has been through this. I have two program switches on my OSAP record.
2016: Left Kinesiology and went on to complete a Psychology degree in 2018.
2019: Withdrew from Sheridan after one semester. Appeal was granted and I was officially deregistered.
Now starting at Mohawk (for medical radiation sciences) and OSAP flagged me for Academic Probation. I get why the system flagged it since both situations were handled properly at the time.
My question is whether Section A needs a transcript or if the appeal letter from Sheridan (to deregister from the program) is enough. Anyone dealt with something similar?
I'm super super super confused.
Idk if I should just call Mohawk to confirm this. I don't want to mess up my OSAP application. I need to apparently fill this Academic Probation document and hand it in, but Section A is confusing.

Do any of the following statements apply to you?
You are currently attending a different school from the institution that you
attended while you were on academic probation;
• You are attending a private institution in Ontario;
• You are attending Dominican University College, Michener Institute of Education
at UHN, Royal Military College of Canada, Niagara Parks School of Horticulture;
or
• You are attending a school outside of Ontario.
Yes – Required documents: You must include a copy of your postsecondary
transcripts from your probation period.
No - Transcripts are not required."
