Here's a scenario. Reply with what you would do FIRST and why — not just the answer, the reasoning behind it.
You're caring for a 72-year-old male, 2 days post-op from hip replacement. He calls out saying he feels short of breath. You walk in and find:
RR 28, labored
O2 sat 89% on room air
HR 118, BP 142/88
He's anxious and restless
Left calf is swollen and tender
What's your first action and what clinical reasoning got you there?
