Absolutely begging for help. Perioral dermatitis, rosacea type 2, or folliculitis? Or something different?

I really have no clue what this is. This first happened in 2021 at the END of my accutane course. My Derm had no clue what it was. So I went to a different Derm and she said it was folliculitis. Tried an oral antifungal for weeks didn’t see a difference then took minocycline. It worked. But only while I was taking it. I found Mandelic acid helped a lot with it! I got pregnant months later never had an issue the whole time I was pregnant. Fast forward a year or so later around my period I started getting the bumps again. I brushed it off as hormonal acne and got on Spironolactone. That helped! I’m still on Spironolactone to this day but the bumps kept coming so I did another round of accutane October 2024- April 2025. All summer of 2025 my skin was perfection. Besides a weird flare after spending the day at the beach pustules erupted within hours. I immediately thought ok it’s Folliculitis so I got it under control in less than a week I think. I’ve been clear of it since then until April 15th 2026. And BOY! It’s been relentless!!!!! I can feel when I’m about to breakout with these pustules just because of the INSANE itch that happens as they’re starting to erupt. The itchiness seems to be the worst on my forehead and nose for sure. They also spread all over my face and the bumps pop up within HOURS! It’s insane! When I pop them (I usually don’t, but I always psych myself into thinking I have hormonal acne again) nothing ever really comes out just fluid but not an actual pus filled zit. These bumps usually get very inflamed but don’t usually hurt unless I pop them then they’ll be tender or almost blister like. Really it’s the itch that drives me crazy!!! It also tends to make me peel a lot as the blisters/pimples start to pop up everywhere too it’s really odd. and my eyelids get dry and itchy as well and sometimes my actual eyes get bloodshot as well! And these bumps usually pop up right around each other and sometimes form like a line?? Currently using a rosacea triple cream and taking minocycline but am being referred to a new Derm because my old one once again does not care and has never bothered to do that biopsy or scrape test thing. Has anyone ever had that done on your pustules? I think just knowing for sure what this is would give me the biggest relief of my life. Heat and sweating is the biggest issue for these bumps. I can count on a flare every time!! Does this look like pustular rosacea? Or maybe not? If it does should i or can I use my retinoid too? These photos are in the span of a few weeks just to be clear.

Not even sure what this. Perioral dermatitis, Type 2 rosacea, or Pityrosporum Folliculitis?

This all started at the end of my first accutane course in 2021. I’ve been dealing with not even really sure what it is anymore and no Derm seems to know either or they just flat out don’t care. I’ve tried everything. These bumps look like zits but have nothing but clear fluid in them, never starts out as a blackhead or anything. It’ll start flesh colored and within hours it turns very inflamed. Antibiotics, 2 Rounds of Accutane, celluma nova panel consistently at home, hypochlorus acid daily, regular retinoid use (stopped it for right now during this flare) rosacea script compound cream, prescription antifungals oral and topical, aerolase, nizoral, BPO wash (always worsens it), Mandelic acid (usually this one is pretty helpful), azealic acid 15%, ivermectin, changing pillowcases daily, pimple patches, prescription sulfur face wash, clindanycin, mupirocin inside nose and on spots, tacrimolus (I know I butchered the spelling probably lol), I’ve tried it all. I’m sure there’s more I can’t even think of m. But anyways ive randomly had a major MAJOR outbreak of it once again 🥴 so bad that in the last 24 hours its changed from getting maybe 2-4 pustules daily to probably 10+ new pustules in the last hour. I know I caused it to erupt because for one I was touching my face a lot yesterday but then also used a Mandelic acne face scrub, yes I know that was the most idiotic choice on my part. All that being said…for it to spread the way it does and rapidly is likely some form of folliculitis? I’ve been told that before by an amazing Derm, but then told rosacea by another. Chat gpt says it’s a mixture of Both. Idk anymore but I’m feeling SOOOO down. It’s been along time since I haven’t been able to get the pustules under control. The pictures aren’t showing up how many sadly but when I touch my face it feels like hard blisters everywhere. They don’t hurt, but they do itch. If I pop them it obviously spreads rapidly and will burn some. It takes each spot FOREVER to go away just for a new one to cluster up right next to where the old one healed up. They also tend to like to form in a line or cluster
