the manga is about a doctor that goes to another world thru a fog when he goes to a remote island, the manga had brief but detailed explainations of the procedures he perform, in one of the chapters he performs a stomach surgery to a dragon from inside, and basically he starts gathering people to learn form him, elaborate anestesia, etc.

Maybe it’s just overthinking, or maybe it’s a theory, let me explain:

Gohan had anger issues as a kid. There were two occasions where his anger took over and he went straight on the attack: the first against Raditz, and the second against Frieza. When Gohan turned SSJ2, we saw a streak of light crossing his head (kind of like what we saw when Goku turned into an Oozaru in Dragon Ball). After transforming, Gohan’s personality showed an almost complete shift, as if something else had taken over him.

Maybe all of these were loose hints that led to Gohan’s most recent form.
