Should I be worried about this newly formed spot? early 20s and have tons of weird moles but they're all small

I just noticed this on my shoulder/arm and since I've been trying and failing to get a derm appointment for years I wanted to ask here whether this is something to worry about. the grey ring around it is just where I marked it with pencil so It'd be easier to photograph.
I don't get a lot of sun and usually have my shoulders covered, I have undiagnosed skin problems like very dry and sensitive skin and I suspect psoriasis. I have tons of weird looking moles that have different colors or aren't round or look lighter in the middle or have tiny dark brown or red spots but they're all very small, I've tried many times to get an appointment to get them checked out which is covered by insurance but in my country a mole checkup doesn't pay well so derms refuse to give people appointments for that. I could try to get one by saying I'll pay more out of pocket but I wanted to ask whether it's urgent first.
