Tirzepatide stopped working after switching from Brello to Pomegranate — dosing help?

Has anyone else switched from Brello → Pomegranate and felt like their meds suddenly stopped working?

I’m mainly trying to figure out the right next dose, so I’d really appreciate input — especially from anyone who’s had to re-titrate or switch formulations.

Context:
I had to switch from Brello because they randomly stopped serving Michigan. I actually really liked them — great customer service and a super clean UI.

Pomegranate has been… not great so far. The UI is clunky, support has been pretty unhelpful, and when I asked my provider about dosing, I basically just got: “you can take up to 7mg”… which wasn’t helpful at all given my history.

Important difference in meds:

  • Brello: tirzepatide/B6 from Southend Pharmacy (22mg/1ml tirz + 4mg/1ml B6)
  • Pomegranate: tirzepatide/B6 from BPI Labs (10mg/1ml tirz + 10mg/1ml B6)

Same “mg dose” but it feels way less effective.

My full titration history:

(took 1 week off)

Then switched to Pomegranate:
4.84 → felt nothing
5.1 → felt nothing

Before the switch, I had solid appetite suppression and was losing weight at ~4.8mg. Now I feel zero effects and have actually gained weight.

Also worth noting: I haven’t had any nausea recently, which makes me feel like I can increase — I just don’t want to overshoot and end up back where I was at 5.94mg.

My main question (dose-related):

What would you do next in my situation?

  • Jump to ~5.5mg?
  • Go to 6mg?
  • Stay at 5.1mg longer?

Secondary question (longer-term / pharmacies):

I also just got a notification that Pomegranate is no longer offering BPI Labs. I still have about 2.5 months of BPI meds left, but now I’m trying to think ahead.

  • What pharmacies are people having the best results with right now?
  • Has anyone switched away from Pomegranate and had a better experience elsewhere?
  • Is it worth sticking it out or jumping ship?

Would really appreciate any insight — especially from people who’ve had to adjust after switching providers or formulations 🙏
