LVP and slab moisture

I have a 2015 slab home in North Carolina. I had new CoreTec LVP installed in 2023. Recently during a minor remodel, some of the LVP had to be removed, and there is extensive mold and moisture under the LVP. There has been no flooding. I am not in a flood zone and I do not carry flood insurance.
I suspect that the slab moisture is escaping, and getting trapped under the waterproof LVP flooring and creating this damp environment. Obviously it all needs to be torn out now. I am hesitant to go through insurance, as I worry that they’ll hear “moisture” and “no flood insurance” and deny … even though there’s been no flood. I also worry that in addition to denying … they will hear “mold” and it’ll cause insurance issues in the future.
The floor installer says they installed correctly and “they’ve never seen this before”. The manufacturer does not state expressly that it NEEDs a vapor barrier - it says it is up to the flooring contractor.
I’m at a bit of a loss as to which way to go now. Ideas? (Please be constructive - I’m really emotional about this at the moment)
