Chronic fissure since November – diet good but still bleeding, is surgery my only option?

Hey everyone,

I’ve been dealing with an anal fissure since November and it’s been really frustrating.

I had bleeding episodes:

3 times in January

1 time in February

and now twice on back-to-back days in April

In the first 1–2 months, I was using sucralfate-based ano creams, and later I switched to nitroglycerin (Nitro) ointment and a 2% gel.

The confusing part is that my diet has been pretty solid throughout:

high in soluble fiber

drinking around 3–4 liters of water daily

But despite this, my stools still don’t stay consistently soft, and I keep getting flare-ups.

At this point I’m not sure what I’m missing.

Is this something others have experienced even with a good diet?

What else can I try before considering surgery?

Is surgery the only option at this stage, or are there other treatments that worked for you?

Would really appreciate hearing from people who’ve dealt with something similar.
