Hi I am a 21 yr old Female who is approximately 5’11 and 274lbs. I have had a quick onset of suspicious symptoms that are concerning. Symptoms have progressed the last 2-3 weeks only getting worse. Pain started in my left shoulder/collar bone/breast as well as the entirety of my upper back and neck. I had 3-4 small lymph nodes that were swollen, painless and rubbery. In addition to pain I had been having drenching night sweats for 2-3 weeks prior, nausea to the point I was unable to keep down most foods, weight loss without trying (my usual consistent weight range is 288-297) severe fatigue. Initially I went to the urgent care where a chest xray was performed and basic metabolic labs were drawn. No findings on the Xray, and slight elevation of inflammation markers shown through labs. I was advised by the provider at the time to take ibuprofen as an anti-inflammatory medication and that the pain should go away. Pain progressed for the next 2-3 days. Ibuprofen was not helping with pain, I was still experiencing all the prior symptoms as well as a progression of shortness of breath. Checking oxygen levels at home they were stable in the 96%-99% range as normal. As well as a new lymph node swell on my collarbone that was swelling more and more throughout the day. Ultimately I went into the ER that day where I had more labs taken, a urine test, and a chest CT angiogram. I was given IV morphine for pain which helped for a short amount of time before the pain returned. The Chest CT angiogram came back normal. My labs came back mostly normal with the exception of a slightly raised WBC, Lymphocytes, and Neutrophils which were not too elevated for concern. My urine test came back abnormal but according to the ER provider it was unclear whether it was a UTI or not but that it could be the cause of all my symptoms. Regardless they prescribed an Antibiotic to treat my symptoms and sent me home with antibiotics, and nausea medication. At home the pain and nausea medication was not helping the pain for more than 2 hours at a time. The next day I woke up with a severe migraine and pain out of control. I took the pain medication as prescribed and migraine medicine (since I have had frequent migraines in the past). That day we went to visit my sister and my shortness of breath and pain was still a very severe 7-8. Later that day while taking a nap (due to fatigue) my oxygen dropped to 85% and would not come up even when awake and sitting upright. My pulse had increased to the 120-140 range and eventually after a long time of concentrated breathing I was able to bring my oxygen levels to 92% which was not consistent. I went into the ER again due to the pain and decrease of oxygen levels (different ER) where I had a Chest Xray, and Labs done. Xray came back normal, labs were similar to the labs done the morning prior. I was given a muscle relaxer and steroid prescription to take alternative to pain medication. As well as a new nausea med. The next morning pain persists, oxygen is better but is still only ranging 89%-93%. Two days later pain is relatively the same, pain meds, the steroid med, and muscle relaxer was not helping for the pain. And due to the fatigue they gave me with no relief of pain I stopped pain meds entirely. I saw my PCP and was told it could be internal shingles. I was given an 7 day anti-viral medication. And told to follow up with them in 3-4 days if nothing has changed. It has been 3 days with no improvement. I am still currently taking the antibiotics, anti-viral medication, and steroid. My lymph nodes have stayed swollen and the pain has only gotten worse, with pain bilaterally throughout my shoulders/collarbones/upper and middle back with pain spreading to my lower back. I am having severe discomfort in any day to day tasks. I feel extremely weak throughout my body and am concerned what is going on is being pushed off. Can anyone give advice as to what this is? Or what to ask for in further imaging/testing? Thank you!
