When to use azelaic acid and toner in my routine?

I’ve had significant reactions to tret, salicylic acid and glycolic acid over the last decade so I went all the way back to a really basic routine with no actives.

Since having endometriosis surgery in 2023, my hormones have been a rollercoaster and I’ve started getting both cystic acne again and lots of clogged pores and bumps.

I’ve managed over the last three weeks to introduce azelaic acid (from Q&A) into my routine. It’s been purging but so far not much irritation so fingers crossed.

My routine currently:
Cetaphil gentle skin cleanser
Q&A azelaic acid serum
Byoma phyto-mucin serum
Either cerave moisturiser or E45 30 SPF moisturiser depending on AM or PM

Since I’m still getting some irritation (some small, red, dry bumps mostly around the perioral area), I was contemplating adding in the Byoma milky toner.

My question is, do I used the milky toner straight after cleansing and then do the acid serum? Or the other way around?

Is this just acne or is there something else going on?

35y female with endometriosis and other chronic illnesses)

I’ve always had hormonal cystic acne, but the skin in between the nodules was always pretty okay. Over the last few years that has changed (I’ve also been through lots of different hormonal treatments).

It now feels like every single pore on my forehead and chin is blocked, and I have cysts erupting every single day. Some are true hormonal nodules that stick around for weeks, but some are caused by the blocked pores.

I’ve was on tret for around 12 months and although it did improve my skin texture, it didn’t clear it completely and also caused major skin irritation.

I’ve tried salicylic and glycolic acids but my skin is just too sensitive. Even micellar water breaks me out in a rash.

I’ve been using an azeliac acid serum morning and night for around three weeks so far and what I hoped was just purging is still sticking around so now I’m wondering if it’s going to actually work at all.

Current routine:

AM
Wash with warm water
Wipe breakouts with hypochlorous acid
Q+A Azelaic Acid Balancing Serum (wait 5 mins)
Byoma Phyto-Mucin Glow Serum
E45 Daily Protect Moisturiser (30 SPF)

PM
Wash with warm water and Cetaphil Gentle Skin Cleanser
Q+A Azelaic Acid Balancing Serum (wait 5 mins)
Byoma Phyto-Mucin Glow Serum
Cerave Moisturising Lotion
Zinc Ointment + pimple patches to bring down inflammation on the large nodules

£2k David Attenborough painting by Eastenders actor/Geico Gecko Jake Wood seems fishy to me

Eastenders actor (and the Geico Gecko for US Redditors) Jake Wood has recently taken up painting and has a gallery show in my city.

He has painted this portrait of David Attenborough for his 100th birthday and it just seems off. It says it’s mixed media on Canvas, and his other paintings do have collage elements.

The original has been sold for £2k and there are prints for £450.

First thing that threw me off was the inconsistency of the leaf pattern on Attenborough’s jacket. And looking closer there’s the gorilla’s face and David’s hand that don’t seem right.

I hope this is okay to post, I just found it very interesting and don’t really have anywhere else to share it!

A number of years ago, I took part in a university study looking at genetic causes of depression and anxiety. Every so often, they get back in touch with follow up studies that I can take part in. They’ve never shared my genetic data with me, or given me individual results from the studies.

I’ve just taken part in another one to do with habits and behaviours, specifically looking at polymorphism of the COMT gene.

Now, I know absolutely nothing about genetic science but I couldn’t help looking up about this polymorphism I have which apparently makes me more susceptible to anxious behaviour (I have a diagnosed anxiety disorder and PTSD).

According to Psychology Today, COMT activity can be slowed by excess estrogen, and then COMT can’t remove harmful estrogen metabolites, which leads to more estrogen, which leads to slower COMT and so on and so forth.

Knowing that endometriosis is (maybe?) caused by or can cause excess estrogen, could this genetic polymorphism mean that I am genetically more susceptible to developing endometriosis?

Looks like the jury is out according to studies in the area but learning more about my own genes has deepened my fascination with the genetic background of this disease!
