First Warning Letter- RTO non compliance

Hi, I have got this mail with the subject 'First Warning letter' and they have mentioned that

with reference to the counselling letter shared with you, it has been observed that you continue to be non-compliant.

We have found you to be in violation of our policy - refusing to work from base location for February.

Please consider this as first warning and refrain from doing so which will lead to disciplinary action.

My question is - I actually did not go during April due to family health issues. However, why are they mentioning February again and again?

Also, I know some people who are missing RTO in between sometimes but they don't get this warning mail. Then why me?

And since I have reason for missing RTO, should I reply to this mail or send a mail to HR explaining the same? Just to have a paper trail?

Also, the mail mentions that I have missed RTO in February. So does that mean I will get another mail for March and then they will terminate me? Will they even check that I started going later ?
