What’s the biggest "I didn’t realize it would look like THIS" mistake from your floor plans?

I’m really curious to hear your stories. What’s something that was technically "drawn" on the 2D plan, but you (or a client) didn't actually realize was a problem until you stood in the physical room?

Homeowners/Architects: What’s something that looked great on a 2D plan but was a disaster in 3D?

I want to hear your horror stories or would have been great not just good if we discover this early stories?
Tell me about the "surprises" that cost time or money to fix because the 2D drawing was technically correct but practically confusing.
