Can’t figure this out…

I’m looking for ideas or advice because I’ve been dealing with ongoing urinary/bladder-related symptoms for about 3 years now, and they seem to be getting worse and more consistent.

Timeline:

  • The first 1–2 years, the symptoms would come and go. I could have bad periods for days or weeks, then it would mostly disappear for a while.
  • Over the last year especially, it has become much more steady with fewer “good nights.”

Main symptoms:

  • Frequent urination at night (mostly nighttime only)
  • On bad nights, I wake up every 1–2 hours needing to urinate
  • On better nights, I wake up about every 4–5 hours
  • Sometimes it feels hard to urinate, like I have the urge but only a little comes out
  • I don’t really have pain urinating other than it feels difficult and like it takes awhile to get there
  • I often get a dull pain on my left side that feels like it could be bladder-related, lower stomach/abdomen, or possibly even toward my back
  • The pain seems worse when I hold urine too long at night or first thing in the morning

During the daytime:

  • Symptoms are usually less severe during the day
  • Urinating feels easier during the day
  • I don’t seem to go nearly as often during daytime hours

Medical workup so far:

  • I’ve seen a urologist
  • I’ve seen a kidney doctor/nephrologist
  • I’ve also been to my regular doctor multiple times
  • I’ve had various testing done over the years
  • In the past there was mention of “casts” found in testing

What confuses me is that the issue is mostly nighttime-focused, comes in waves, and doesn’t feel like a typical UTI.

Has anyone experienced anything similar or have ideas of conditions that could cause this pattern?

Recently I have found I have low testosterone as well but in medication to get it higher and it’s working but still dealing with these issues.

any ideas would be very grateful where I’m truly struggling and just tired of this..
