How bad are my mollars?

Recently, I finally got over myself and, after many years of neglect, made an appointment with the dentist. He performed a checkup and gave me a referral for an OPG. He didn't mention how bad the situation was, perhaps to avoid scaring me. I received the X-ray and was horrified by what I saw. My lower molars were particularly noticeable. I see huge shadows on them. I have another appointment next week, but I can't stand the suspense, so I wanted to ask you, how bad is it? Is this something that can be saved with an RCT, or is extraction the only option? So far, there's no pain or sensitivity.
I know OPG isn't used to diagnose tooth decay, but I don't have bite-wing X-rays. I don't think they're done that often in my country.
