Tore the absolute heck out of my ACL again. Doctor gave me a temp handicap parking permit note. He physically signed the VTR form and told me the prescription will be available via online portal later. I checked the portal and this is the prescription uploaded, he included his state license ID and contact information with a stamp but no actual signature on the line. Is this technically still valid to take to the DMV? His office said it would be fine.
