Please give me your opinions: should I have surgery?

I’ve been in this journey for three years now. For one reason or another, I kept postponing surgery. For example, I had gallbladder surgery. I was a full time carer for my elderly mom. I got c-diff. My workload was too much.
Next week I will be going back for an appointment and it may be that they will be ready to schedule me. But I’ve been having some problems lately.
I take Monjauro and I think this may be why, but I’ve been having terrible stomach problems. I get the runs where I can’t make it to the bathroom in time, and sometimes I vomit. I take psych meds, which have been changed around a bit an this diarrhea has gotten a bit better but not completely. Plus I’m constantly exhausted. I can hardly get up or do my work. I don’t know why. I was doing good with the Monjauro for a couple of years, but I don’t know what else it could be because I hardly eat. I can no longer tolerate red meat, dairy products except yogurt and kefir, or anything remotely greasy. I have also developed sensitivity to artificial sweeteners, sugary things make me sick, and the last time I tried a protein shake-Fairlife-it went right through me.
Realistically, I have lost weight but I still need to lose about 70 pounds, maybe a little more. All my weight is in my stomach and my back is killing me till I have problems getting around. I’m about to turn 65.
I’m really worried that if I get this surgery I will start crapping myself even worse and vomit even more. I’m afraid I will be one of those people who are still constantly hungry, and I can’t imagine what I can eat because all I can eat now is beans, crackers, hard boiled eggs and boneless skinless chicken thighs. My stomach is sore almost all the time.
I really don’t know if surgery will make my problem worse or better. They have mentioned that they may advise me to continue with the Monjauro after surgery. (I already tried Ozempic but that made me so sick I couldn’t get out of bed for days, except to vomit. It was like a weeks long hangover.)
