Faulty crown?

I have a large crown from a dentist I've been seeing for years - originally placed about 10 years ago, then redone 3 years ago alongside a root canal. From the start, I told him the tooth felt "still alive," but nothing showed up clinically, so we agreed to watch and wait. The crown fell out today, and I'm now likely facing an implant due to insufficient remaining tooth structure.

I'm seeing the dentist tomorrow, but wanted some insight beforehand - could there have been an error on his part? I thought crowns are generally expected to last much longer, and this next step will be both invasive and expensive. I'm also puzzled by how visibly dirty the inside of the crown looks - does that suggest it wasn't properly sealed or fitted to begin with?

Recommendations for counselors for teen

My 15yr old has been struggling with the challenges of teen life, and I feel he could benefit from support beyond what I can provide as a single mom. Dad is present but dealing with addiction issues, so he unfortunately is not a reliable support or role model. Any recommendations for someone who specializes in working with teenage boys?
