I'm trying to figure out if I'm dealing with a healed fissure scar, a pelvic floor issue, or a mix of both.

About 1-2 years ago, I had some minor bleeding and pain during bowel movements. That eventually stopped after a few weeks or so, but it left me with a chronic issue on the right side of my anus.

I don't feel any bumps. Instead, running straight up and down on my right side (about 1-2 inches long), I feel a thin, slightly raised line in my anus. It feels similar to the frenulum on the inside of your upper lip. Touching it directly causes a mild version of the pain I feel when sitting.

Symptoms:

  • When I sit, I get a pain almost like a canker sore where that string is, and sometimes some pain in my upper thigh or even my right hamstring.
  • The pain improves when I stand or lean heavily to my left. Heating pads also help.
  • For some reason, if I pass a very large stool, it might cramp/hurt for a second, but then I feel better than average for the rest of the day. It's like the stretch relieves the tension.
  • I am not constipated, I go at least once a day, and the BMs are only very mildly painful.
  • I have an old tailbone injury from years ago that I was able to heal by sitting on a donut pillow for about a year, then right after the tailbone pain healed, this pain began. And occasionally I have trouble passing gas (it feels stuck deep inside). My pee stream will sometimes stop and start for just a second.
  1. Has anyone felt this smooth, vertical "string" or ridge inside? Is this scar tissue from an old fissure, or is it a tight muscle from pelvic floor issues related to my tailbone?
  2. Was anyone able to fix this?

Any insight would be hugely appreciated.
