Feeling "Out of It" & Suboptimal: Imbalances In Estradiol (E2), Prolactin, SHBG, and Low Free T despite good Total T.

Hi everyone, I'm new. Im 32 and since more than a year I've been dealing with lack of energy, no motivation, lack of "drive," absent libido, and ED. Not even daily Cialis is helping at this point. I also go to the gym 3-4 days a week and do some cardio at the end.

I'm looking for advice on how to optimize this panel. To be honest, I feel like my existence is just 'ornamental' right now—I’m physically here, but I have no drive, no spark, and no 'manhood' left. It's devastating.

Questions

  1. The Free T Paradox: My Total T is 731, but my Free T is at the literal bottom of the range (15.10). Is lowering E2 and SHBG the only way to "unlock" my own testosterone?

  2. Prolactin at 21.0: Since I’m over the range, could this be the main reason Cialis is failing me? Should I jump straight to P-5-P or consider something stronger?

  3. Protocol Sanity Check: My doctor wants me on 3mg of Anastrozole/week (Monday, Wednesday, Friday). which sounds like a massive overkill and I'm afraid of crashing my E2.

I’m leaning towards 0.25mg twice a week instead.

- Does this sound like a more rational starting point for someone who isn't on TRT?

- Has anyone fixed their ED/Mood by lowering E2 and Prolactin with these numbers?

Localized Cavernous Fibrosis/Fibrotic Nodules in the Left Cavernous Body, any alternatives (besides going for the implant)?

Hey everyone, it seems this is the end of my ordeal, or at least partially, friends. The doctor only gave me medication to reduce this damaged tissue, and just hope for the best so that this can be reversed in the long run.

As an aside, I also had a hormone panel done because apparently my hormones were deficient. The doctor told me that my E2, Prolactin, DHEA-S, and Free Testosterone levels were suboptimal, and that this is probably underlying the problem.

* Hormonal Panel Main Data

  • Prolactin = 21.0 ng/mL.
  • E2 = 42.9 pg/mL.
  • DHEA-S = 231.3 µg/dL.
  • SHBG = 40.90 nnmol/L.
  • Total T. = 7.31 ng/mL.
  • Free T. = 15.10 pg/mL.
  • Bioavailable. T. = 3.30 ng/mL.
  • Cortisol = 16.6 µg/dL.

I originally started with low libido and poor erections, but I got this from a masturbation injury. I didn't address it in time, and now I'm dealing with a bunch of things that are making my life miserable.

The urologist prescribed Anastrozole to lower my Estrogen first and release my Free Testosterone; then they would proceed with the Prolactin. And the medications for fibrous tissue, well, they were commercial products, he prescribed Vitamin E and Colchicine, but said that this wasn't fast or anything, he said that even things like PRP or Exosomes aren't 100% safe, and they're quite expensive., but that optimizing the hormonal issue could also accelerate the process, even if only a little (he probably acted somewhat optimistically about this too, but the thing is, I don't have anywhere else to turn, so it is what it is.).

Do you have any advice or alternatives that you know of that could be complementary for a case like mine?
