I recently had a periodontal screening, and the results were a harsh reality check: 100% plaque index, 90% bleeding on probing, and early signs of periodontitis (deep pockets).
Like many Aspies, dental hygiene has always been a massive struggle for me. But my main issue isn't just executive dysfunction or sensory overload. My biggest barriers are a complete lack of fine motor skills and zero spatial awareness.
I physically cannot maneuver a manual toothbrush at the correct angles. I tried using an electric sonic toothbrush to compensate for the motor skills, but because I lack spatial awareness and proprioceptive feedback, I ended up pressing way too hard and doing more harm than good. I just can't feel or visualize what I'm doing inside my mouth.
In my country (Germany), public health insurance only covers deep periodontitis treatments if the patient shows "compliance" – meaning you have to prove you can maintain a strict at-home brushing/flossing routine. Since my neurological reality makes this literally impossible, I decided to be completely honest with myself and my dentist. I just canceled my standard treatment plan.
Instead, I am opting for delegation. I plan to stop forcing a neurotypical at-home routine that I will inevitably fail at, and instead pay out of pocket for frequent Professional Dental Cleanings (like every 2–3 months) so the hygienist can do the job I physically cannot do.
Has anyone else taken this route?
I'd really appreciate hearing your experiences. It feels weird to go against the standard "just try harder to brush" medical advice, but I need a solution that actually fits my brain. Thanks!
edit: I do not plan to give it up entirely just cannot provide the compliance.
