Hello! My girlfriend recently applied for a PWD ID in Mandaluyong City Hall. Just sharing her experience for your information.
FAQs:
Gaano katagal ang process? I arrived 9:28 am and had the ID by 10:15 am.
Gaano katagal ang validity? 5 years. Nag-provide din sila ng booklet for logging yung purchases ng meds.
Requirements:
The ones I presented are as follows:
- Medical certificate with diagnosis and note from Doctor that I am eligible to possess the PWD ID privilege. Pwede rin ang Certificate of Disability.
- Proof of residency. Since I am renting, pinakita ko yung card sa condo and valid ID with address sa Manda.
- ID photos 2 pcs 2x2 and 1 pc 1x1
- Possible din hingiin ang PhilHealth ID pero hindi ko sure kung mandatory.
Required po ba na voter ng city? No, as long as may proof of residency, okay na yun. I also am a permanent resident sa province which wasn’t an issue. Though, be prepared lang since they asked me gaano katagal na ako sa Manda.
Observations:
- Generally a good experience, maayos na inexplain ng nag-assist sakin yung form and questions, mostly personal info including family background (Names, Birthdays, Household monthly income, etc.)
- Airconditioned ang office and mabait ang vibes ng mga tao.
- Well-explained din yung privileges bilang ID holder.
