Need help with what to do, dermatologist gave me cream but it makes it worse or does nothing

In weird spots on my hands. Seems like anything I try doesn’t work. Have tried completely leaving it alone and nothing happens. When I put any sort of vasoline, excema cream it makes it worse. No clue what is making it flare up. I work at a shop where I’m handling money so I do wash my hands pretty frequently and use hand sanitizer. PLEASE HELP. I feel so embarrassed when my hands look like this

I’ve had excema my whole life
22 now and still have it, not as bad but in bad spots
My hands get it and it looks super bad.
It’s in weird spots and it’s normally the same spots
It will flare up then go away if I don’t mess with it. The cream the dermatologist gave me made it worse
Normally get bad after I shower then won’t go away for weeks
