Adderall Feels Life-Changing Sometimes, Then Randomly Makes Me Feel Flat and Terrible

I’m 21M prescribed Adderall IR 15mg 3x/day for ADHD/anxiety/brain fog issues. Sometimes I take it 2 or 4 times a day.

I work a pretty intense sales/business job where I need to be social, sharp, confident, focused, and talking to people all day. I also lift heavy consistently, played sports my whole life, and try to stay disciplined with sleep, food, hydration, etc.

I also drink caffeine pretty regularly (coffee/pre workout/energy drinks sometimes), especially because my job and lifestyle require a lot of energy/output throughout the day.

The confusing thing is when Adderall works WELL, it genuinely feels life changing. I feel:
- socially smooth
- less awkward
- mentally quick
- motivated
- emotionally present
- more confident
- calmer instead of overthinking everything

It feels like it fixes a ton of issues I’ve dealt with my whole life.

But then randomly I hit these periods where it suddenly feels terrible instead of helpful. I’ll get:
- emotional flatness
- irritability
- stomach discomfort
- bad crashes
- feeling socially frozen/awkward again
- feeling mentally slow and numb
- overstimulated but still exhausted somehow

Sometimes taking more or adding caffeine helps temporarily, but other times it just makes me feel worse and more physically uncomfortable.

What confuses me is how inconsistent it is. Some days it feels smooth and almost therapeutic, other days I feel emotionally dead and physically stressed.

Also recently lowered my SSRI dose, so idk if that could be affecting things too.

Has anyone else had this pattern where Adderall works amazingly for a while and then suddenly starts making you feel flat/bad? Did you figure out what was causing it?

21M — Clinic Recommended TRT + HCG After Repeat Low Testosterone Labs, Need Opinions

21M considering TRT + HCG and looking for honest opinions/experiences from people who’ve been in a similar situation.

I’m 5’11.5”, 190lbs, played sports my whole life, lift heavy consistently, have a solid amount of muscle, eat pretty healthy, and I’m not overweight, so this surprised me, but it makes sense considering other symptoms I’ve had over the years.

On December 1st, my labs came back:
- Total T: 363
- Free T: 7.8 pg/mL

At the time, I thought it was probably because I wasn’t eating enough while training heavily and was in a calorie deficit for a while, so for the past few months I intentionally started eating more, getting into a surplus, focusing on recovery, and increasing healthy fats before retesting.

Yesterday’s labs came back:
- Total T: 305
- Free T: 69.5 pg/mL

These really concerned me and I visited a recommended men’s health clinic nearby where they talked to me about symptoms and risks.

The clinic I went to recommended:
- TRT 150mg/week
- HCG 500 IU

They basically told me it’s a pretty common protocol for younger guys and that fertility risk is relatively low with HCG included, but I still want outside opinions before jumping into it at 21.

Main symptoms for years have been:
- really bad brain fog
- flat mood/emotional blunting
- anxiety/social anxiety
- feeling mentally “slow”
- low confidence
- feeling emotionally numb at times
- overthinking/self-monitoring constantly

I’ve also had very bad anxiety throughout my life in general.

I’m prescribed Adderall IR 15mg 3x/day, which honestly helps those symptoms a lot socially/focus-wise, but lately I’ve also been getting harsher crashes, irritability, stomach discomfort, and feeling super flat again.

I’m also on risperidone 1mg for sleep

I’m honestly really considering TRT and think I’m probably going to do it, but I want to make sure I’m not making a dumb long-term decision at 21. My biggest concern is fertility because I definitely want kids in the future. I’m thinking about getting a semen analysis before making a final decision.

Questions:
- Do these levels/symptoms sound TRT-worthy to you guys?
- Did TRT actually help brain fog/mood/social confidence?
- Anyone regret starting this young?
- How much did TRT + HCG affect fertility?
- Could meds like SSRIs/risperidone be contributing more than I realize?
- Is 150mg considered high for a starting dose? What about 500 HCG?

Would appreciate honest opinions and experiences, especially from younger guys or people who started with borderline levels.
