I’m so confused

As you may deduce from the picture, this is a cycled, established 30 gallon tank of almost 3 years. I don’t do a full test super duper often maybe once or twice a week because I have had several tanks and once the levels are good I just let it go and keep an eye on nitrate. However as you can see all three levels are off, drastically in my opinion. I did a 50 change yesterday because the levels were so drastic. I’ve had this axolotl (baby hamdog sandwich jr) for a year and some change in this tank that WAS CYCLED!!! I added that piece of wood that’s behind him about a month ago I don’t know and don’t think that would be an issue and I baked it before I added it. In my opinion my axolotl does not look unhealthy, he flares up and gets purpley red in the gills at that time and gets super fluffy. He is active at night and swims well, eats well. I’m just so concerned about this. Please help!
