A minion projectile that just missed Sion in his passive (zombie mode), resumed to follow him to the fountain after respawn ... It damaged him, and removed his Homeguard MS bonus
The same thing happened to me a long time ago (probably over a year) with a Kalista AA. I reported it through the support system then, but it's still a thing.
