So I went to a physician 8 years ago for a case of mono, when I look back in my medical records that visit is well documented, there are dates for the diagnosis and corresponding prescriptions as well as when they were filled. However on that same visit there is an unexplained Bipolar 1 diagnosis that has no date of diagnosis, no corresponding prescriptions, not even an explanation as to what, where, or why it is there in the diagnosis tab. I've reached out to the practice but they refuse or are unable to explain it (they seem just as confused as me). I've been to a licensed psychiatrist who specializes in BPD and similar disorders, he reviewed my medical history and examined me and also agrees that I don't fit the criteria for any sort of mental illness/disorders. I've never had any sort of mental health issues and it only appears in my history with this one visit, the only reason I found out is because I went to join the Army and the MEPs doctor looked at me very confused and said something along the lines of "What the hell is 'Doctors note, BP?'" and it's now preventing me from perusing career opportunities.

I've reached out to the licensing board who said they can't help, I've spoken to lawyers who all tell me this is outside of their scope or that it has to cause me harm in order to have a case. I have no medical insurance, my career options are limited b/c nobody can afford or is willing to pay a livable wage for what I do in my area and I can't afford to move my wife and myself across the country for work (I work construction and I'm a good carpenter), recently I broke my hand and fear I will lose some function in it b/c I can't afford to go to the doctors (most won't even accept me without insurance), what can I do? A mate of mine told me to write up a HIPAA amendment form and get it notarized but the libraries near me no longer provide free notaries and I can't afford to drop a hundred dollars on something that might not even work anyways. What can I do?
