Background:
- Diagnosed with severe OSA
- Diagnostic AHI: 41
- Oxygen nadir during study: 67%
- Using ResMed AirSense 11 AutoSet (APAP mode)
Main issue: I am sleeping less and less because I keep waking up from mask leaks/"burping" or "farting" sounds from the mask. It seems worse at higher pressures. I’m also not sure whether I’m waking from leaks, pressure changes, residual arousals/RERAs, or something else.
Important: I made a rookie mistake and changed multiple things at once on the same night after struggling badly with leaks and comfort.
Changes made:
- Changed mask from AirFit F20 silicone cushion to AirTouch F20 memory foam cushion
- Increased minimum pressure from 5 → 7
- Turned ramp off
- Changed EPR from Ramp Only 3 to Full Time 2
After the changes:
- AHI improved
- leaks improved significantly at first
- but median pressure increased a lot
- I’m still waking up and now seem to spend much more time at higher pressures
Questions:
- Does this look like pressure chasing / unstable APAP behavior?
- Could higher pressures be contributing to the mask leaks and awakenings?
- Does anything in the data suggest residual flow limitation/arousals despite relatively low AHI?
- Would narrowing the pressure range or trying more stable pressures make sense?
- Any thoughts on mask fit strategy for F20/AirTouch at higher pressures?
