Hi, this is my first time applying for UC, and typically I’m a bit lost. My background: I am now 35, I was diagnosed with breast cancer in July 2023 and completed treatment for this in May 2024 (chemo, immuno, surgery and then radiotherapy). I have been unable to work since July 2025, the long term effects have really knocked me down. I have been back and forth from the GP for 2 years, there’s really nothing they can do now as all my tests are coming back “normal”.
I’ve filled out my UC50 form, and asked the GP surgery to complete the last page. For details of treatment, they have only put my surgery and not put anything about all the other treatment I’ve had. For “is your patient awaiting/undergoing or recovering from chemo etc” they haven’t answered at all. For the opinion, they have answered no to impact of treatment on work limiting side effects. Ok, this is an “opinion” but they have completely omitted my treatment so has it been considered? Is it worth going back to them to dispute this? I wasn’t sure whether the recovery on this had a time limit and so they didn’t answer. This is not my usual GP who has signed this part of the form, I’ve actually never seen this one before. I will be printing off my medical notes from oncology and sending these in along with the form, as I’m worried how much sway this last page will have. Any thoughts or experiences with this?
