No period for years, now spotting?

Hey all, I've been on T for about 2 and a half years now. I stopped getting my period a few months into starting T and haven't had one since. Alongside that, about a year or so ago I started taking birth control since I'm sexually active and terrified of getting pregnant.
A few days ago I noticed blood in my underwear, which deadass shocked me to my core because I've not had that happen for such a long time. Defineltly not a full period amount, but enough for me to be concerned. Since then, I've not had as much, but have noticed it whilst wiping.
I'm really trying not to think about it being a pregnancy because it's such a dysphoria inducing topic for me. I did miss my birth control a few weeks ago, and have since taken a pregnancy test that came back negatively.
Does anyone know of any reasons beside pregnancy that this could happen? I've tried to Google it but there aren't a lot of resources about Testosterone and periods and how they interact, a lot of the advice is aimed at cis women and isn't relevant.
