Some background. Had braces as a early teen, wore my retainer until late college at which point I got bad about it and stopped, teeth shifted. I am not sure what the diagnosis/treatment back then was and my parents don't remember. I am now 34M.
About 4 months ago at a normal cleaning my dentist noticed some chipping and said I really should go to an orthodontist as I had a traumatic bite. I had noticed my bite starting to feel a little weird and getting some teeth sensitivity. I now have a significant edge-to-edge front bite and really dont like closing my teeth.
Consulted 3 orthodontists. All 3 said I was class III with a crossbite. Two said I was non-surgical, the third said I can do camouflage but she might recommend surgery for complete results. All 3 recommended invisalign. I went with one of the 2 that said I was non-surgical because I got by far the best impression from her. The other 2 spent all of 2 minutes looking at my teeth before talking Invisalign and prices, my current ortho took pics, pano, and ceph and spent a good 20 minutes discussing with me for a free consult. She is very confident she can get good results without any extractions and possibly without even any elastics. I did some research, she has great reviews and has been an ortho here for decades, so I signed up to move forward.
While waiting for the scan and prescription, I started to get somewhat significant TMJD symptoms. Thats when I started to do much more research into TMJD, orthodontics, and jaw surgery. I paused starting the invisalign treatment (I have not started at all yet) to get some TMJD consults. I have had 2 consults with ABOP board-certified orafacial pain specialists and have a third lined up in another week. Both agree I have some minor joint issues but it is primarily muscular and psychosocial (I have anxiety generally and the TMJD symptoms started heavily during a very stressful time) and both agree that orthodontic work is generally TMJD neutral and are not concerned with me starting ortho treatment (both have said they would work with my ortho to monitor my TMJD during the treatment). One of the two docs was pretty wary of jaw surgery as she has seen more negative impacts on the TMJ than positive.
While I do have some TMJD symptoms still, my main daily pain is my bite. It feels very weird, it's causing teeth sensitivity, it is hard to chew and swallow properly, and the front edge-to-edge hurts and is chipping my teeth. I want to move forward with treatment, but I have also found a lot of negativity around class III camouflage here (I guess r/jawsurgery is somewhat of a selection bias). I guess I just wanted to see what people thought. I really want to avoid jaw surgery and I am not at all concerned about aesthetics. My only concern is long term functionality and health of my teeth, jaw, and TMJ.
