Hey all, I'm trying to figure out what my knee pain is being caused by, and thus how to treat it properly. Here are my symptoms and some background.

Background: I’m an experienced runner, I’ve been running 50+ miles/week for 3+ years. I trail run and have pretty seasoned quads from hard downhills. No history of injuries.

I started feeling knee pain on an easy 4 mile run. Ran the next day in the treadmill (uphill only) to minimize pain. It eventually started to hurt again on that run so i rested for 5 days and just biked, as well as took a good amount of ibuprofen, stretched, and massaged my quads. Went on a test run and within 2 miles the pain was back and a mile later I was forced to walk and could only run uphill.

  • Pain Characteristics & Location: It starts as a dull ache in the center of my knee below the kneecap, as the pain progresses it becomes sharp on the inner and outer sides of the kneecap, kind of near the indents but slightly below. I also feel a swelling sensation before the pain gets really intense.
  • Intensity & Triggers: The pain is especially intense during downhill running, where it reaches about a 7/10 level. It is progressive, meaning the longer I run, the worse the intensity becomes.
  • Relieving Factors: I feel noticeably less pain when moving uphill. I have no pain when my knee is fully extended (straight), and the pain completely disappears after I stop running.
  • Localized Tender/Tight Spots: I have minor (1/10) lingering spots of tenderness kind of everywhere on my knee after running. Locations include on the outside of my knee (same location as classic IT issues) at the point where my quad attaches to the top of the kneecap. These spots feel like localized tightness from swelling.
  • No Pain Outside of Running: After running, even just 30 minutes after I can jump, squat, wall sit, etc. with no pain. I can push on my patellar tendon hard with no pain, in fact I can push anywhere to cause pain. I do feel some swelling and thus tightness as described above.
