I've experienced this myself and i saw this in a pro game yesterday too, wildcard vs fisher college:

the player on wildcard pulls out a nade and i assumed he clicks to throw it, but he was surprised it was still in his hand...so he just puts it away lol.

it seems like the button press to throw isn't registering always or u can no longer instantly throw nades when you pull them out.... or something is buggy

anyone else experience this?
