In a lot of plants, I keep hearing the same thing: "The issue gets "managed" but never fully resolved."
People adapt.
Operators create workarounds.
Maintenance keeps certain machines running just enough.
Teams learn how to live around recurring problems.

Examples: bypassing small process issues, repeating the same temporary fixes, restarting the equipment multiple times per shift, etc.

At some point the workaround becomes normal operations.
