How to handle updates to literature when studying for Step 2?
I recently had an older NBME question go something like this:
17yo male presents 1 hour after a 2cm LLQ stab wound which is penetrating underlying fascia with HR: 130 and BP: 110/70.
The options included: CT Abd w Contrast and Laparotomy among others
My concern: Currently, guidelines for penetrating trauma management consider expectant (non operative) management over laparotomy in hemodynamically stable patients with penetrating abdominal wounds. (Reference: Medscape Article Here)
The correct answer in this question was Laparotomy but recent guidelines as far back as 2020 would suggest this patient may be able to tolerate a CT Scan. Even amboss has a flow chart where this patient would get a CT before going to surgery.
So, when does the USMLE change the answer to match current guidelines? Whats the ground truth for us essentially? Im sure this applies to many other questions where guidelines and active research is evolving the field so how do we handle these algorithmic updates?
