13yr MN DMH, roughly 16.5 lbs, down from 17.5 lbs when this started
Indoor cat rescued 2 years ago, previous history unknown, but assume outdoor or indoor/outdoor
History:
Developed diarrhea on April 2, which lasted about 2 weeks, even with a 7 day course of metronidazole.
Appetite decreased on April 4, then stopped eating on April 6. Had to syringe feed at home for about a week, then he slowly started eating some again and now his appetite is up and down, will eat about 25-50% of normal food some days and some days won’t eat and we occasionally syringe feed to supplement. Offering lots of different types of dry and canned foods.
Since April 4 has intermittently had bouts of extreme nausea manifested in profuse drooling. But no vomiting.
April 14, we found him on the floor shaking and shivering and not very responsive, we rushed him to the vet. He had a fever. Treated with onsior and sent home.
Has had episodes of heavy breathing with abdominal effort. Intermittent lethargy.
April 30 anemic on bloodwork (27%). Started on doxycycline. Rechecked May 4 and up to 30%. No improvement in breathing or appetite or energy level though. Has lost about a pound since this started despite supportive care.
Test results:
fPL normal
Chem and CBC normal other than anemia
Ultrasound unremarkable
Vector borne PCR normal
Blood smear unremarkable on pathology
FeLV/FIV negative
Medications: zofran, Cerenia, sq fluids as needed, doxycycline, probiotic

What else can we do for him?? Any other tests to run to try and get some answers? Thank you!
