IMS Milan 2026 - doubts.

Hey everyone,

I’m planning to apply to IMS Milano this year and have a ton of questions. I'd be grateful if any IMS Milan students or IMS students in Italy could share some insight. I've attached a list of questions, and I know that all of them won't be answered, but any information related to my doubts would be very helpful.

First off - do we have to apply to UniMi on their official website or the Universitaly website?

Cost of Living + Scholarships

  • What are the realistic living costs (rent + food + transport + bills) for a student in Milan per month?
  • How hard is it to find affordable housing (monolocale/studio) near Niguarda or the main hospital areas?
  • Are there scholarships or fee‑reduction programs for non‑EU IMS students?
  • Are there part‑time work options (only on‑campus, something like research.) that students can realistically balance with IMS workload in the later years?

IMS program structure and academics

  • How USMLE‑aligned is the IMS curriculum?
  • What is the exam style? I understand that the exams are oral, but are there any written exams as well?
  • How structured is the transition from the non clinical years to the clinical years and are clinical rotations well‑organized?
  • What is the timetable for the day, say, for a first year?

Professors and teaching quality

  • How supportive are the professors in IMS Milano?
  • Do we get feedback on exams, since they are oral? How are we graded, and how is it poissible for us to improve without knowing what topics to work on? Is it mostly pass/fail with limited detail?
  • Are there good mentors who actively help students with career planning or research in the later years?

Lab opportunities and research

  • What are realistic research opportunities for IMS students (summer labs, thesis projects, and publications)?
  • Are there specific labs or departments that take on a lot of medical students?
  • How formal is the research process? Do we mostly reach out ourselves, or are there structured programs?

Practical experience and clinical exposure

  • How early do you get hands‑on clinical exposure like vital signs, history taking, basic skills like drawing blood?
  • In hospitals, do students usually get rotational responsibilities?

Neurology and neurosurgery

  • How strong is the neuroscience / neurology department at UniMi?
  • Would IMS be a reasonable foundation if someone wants to later pursue neurology or neurosurgery‑focused training in the US?

USMLE and long‑term career planning

  • How many IMS students actually plan to take the USMLE and aim for the US/Canada?
  • Is there any formal USMLE prep?
  • Do people feel they can balance full‑time IMS workload with dedicated USMLE Phase 1 prep (e.g., 2–3 months before Step 1)?
  • How does the degree and ECFMG recognition work for IMS graduates aiming for US residency?

Free‑mover exchange programs and ties to US med schools

  • Are there formal exchange programs or free‑mover programs that lets IMS students do rotations in US medical schools or hospitals?
  • Does UniMi / IMS have partnerships with US universities or research institutions that might help with electives or research abroad?
  • How difficult is it to secure a US clinical elective (e.g., 3rd‑year clerkships) as an IMS student, and do you need strong research or connections?

If you attended IMS or are currently studying there, I’d love to hear:

  • What you wish you knew before starting.
  • What you’d do differently if you had to choose again. Would you even choose Milan over other IMS courses?
  • Any pros and cons vs. other Italian IMS programs (mainly Bologna) in terms of research, USMLE prep, and ties to neurology/neurosurgery.
