What makes Tunisia the only African country below the replacement level as they claim

6 years ago when I was in Tunisia I visited a lot of doctors and got Echo, ECG, blood tests done just for them to end up telling me I was having panic attacks and anxiety (I do have them but the symptoms were more than just heart attacks) anyways today I found out I have a severe Cardiomyopathy that has been there for years considering how damaged the heart is, and idk if I should be happy or sad cuz I've been telling them that what I was having isn't panic attacks... Idk what does this mean but I think doctors need to stop sticking mental health to physical health most of the time especially when it comes to these symptoms...
And one more thing, non of them even recommended that 48h heart recording...
