Hello all! I am looking for some input on home health priorities. The client has a fear of falling and refuses to demo a shower transfer and seems afraid by the suggestion. She is afraid of being put in a nursing home. Her dynamic balance is fair (needs contact guard during gait, requires quad cane) and static balance is good on subjective balance scale. Her RUE is affected- half normal ROM of R shoulder, gross and fine motor coordination impaired (cannot open some containers, cans, bottles, has difficulty carrying pre-made meals from fridge to microwave, unable to maintain grasp on items). As a home health OT, would you prioritize her fear of falling first or her RUE function first? I was thinking a LTG goal toward shower transfers w/ decrease in fear of falling could be a good place to start since that is prioritizing safety. The STG could be toward dynamic balance/ trunk control possibly. Any thoughts? Thank you in advance!
