High SHBG best course of action
I was at a fertility clinic for mildly related reasons. Anyways it seems my SHBG is sky high. Seeing an endocrinologist this week.
Which markers would you have check in the lab if you were in my shoes?
Context: 38, male, live in Germany
Symptoms: Low energy and sleepy large parts of the day, basically I constantly wish there was a bed or couch nearby to take a nap. Libido is not terrible, but it used to be quite high in my 20s and it's nothing like that now, which is disappointing. Erection I get even though they take a while and I can usually manage to maintain them for the time needed in the frequency needed, e.g. once to twice a week. Never any morning wood. Can't remember the last time I've had that.
(Edit: I should add, I am 77kg at 1.84m, i.e. healthy with a decent physique due to lots of exercise. But have been gaining a bit of fat in the last 3 years despite decent exercise and nutrition and get quite tired the day and second day after working out.)
Results at this point (they are from a test for fertility)

Testosterone seems to translate to 19.39 nmol/L
SHBG 62.1 - which is very high.
Morning Cortisol was quite low
This was a one time result.
I am not diagnosed with anything (except for ADHD lol). I have also not gone to an endocrinologist yet.
The only other result that was interesting was a liver marker, but it's from two years ago.
| GPT (ALAT) | 45 | u/l | < 45 | borderline high |

Anything to make of it? Which markers should I get tested to get a good picture on Testosterone?
