Retreatment or Extraction?

I got a root canal 4 years ago on #18. Have been having pain on and off for almost a year now, and the cap is wobbling a bit, it was also reseated 2 years ago because it was wobbling. General dentist said there's a gap that could cause infection, and some decay.

They deferred me to specialists but I haven't gone yet, there's a long waitlist. I'm wondering if I should go to endodontics or a surgeon and if you think this could be saved or if I should just go straight to extraction + implant route. I'm 37M, and prefer to save it if possible. I'm starting to have some pain every day now, though it's not extreme.
