





Severe OSA (AHI 63) — 4 months into CPAP, AHI <1 but struggling with mask fit (large nose) and looking for advice
Hi everyone,
I’m a 50-year-old male from India. I was diagnosed with severe obstructive sleep apnea (AHI 63) in December 2025.
I’ve been on CPAP therapy using the ResMed AirSense 11 for about 4 months now. My AHI is consistently below 1 (often around 0.5–0.7), so therapy seems to be working well overall.
For masks:
I started with the Philips Wisp nasal mask
Switched to the ResMed N20 due to cushion availability issues
Tried the ResMed N30i, but couldn’t get a proper fit with my nostrils
Eventually ended up buying another Philips Wisp again
So now I have four masks 😄
The main issue:
The N20 “Large” size still doesn’t feel like the right fit for me (my nose is on the larger side)
The N30i was uncomfortable and I couldn’t adjust it properly
I’ve been adjusting my own settings using OSCAR and learning from this forum since good sleep clinics are hard to access where I live (we mostly rely on pulmonologists). I’m really grateful for the knowledge shared here—it has helped me get to a comfortable setup.
I’m sharing some OSCAR screenshots and would really appreciate feedback on:
Whether my therapy/settings look appropriate
Mask recommendations for someone with a larger nose
Any tips to make the N30i work better (if it’s worth retrying)
Anything else that I am doing wrong here.
Open to any suggestions or improvements.
