After 6 years on TRT, trying what feels like an endless number of protocols, I finally figured out why I so often felt like garbage on testosterone replacement therapy.

For years I suffered from brain fog, mood swings, unstable libido, and just a general sense of feeling off — more often than I felt good. And this was regardless of how good my bloodwork looked on paper.

I tried everything:

- **Injection frequencies:** ED, EOD, E3.5D, E7D

- **Doses:** Ranging from 80mg/week all the way up to 200mg/week

- **Injection sites:** Delts, glutes, ventroglutes, quads

- **AIs:** Both Anastrozole and Exemestane, in various doses and frequencies

- **HCG:** Because I read it helped others with brain fog and libido

- **Needle types:** Insulin pins for shallow IM and subQ, as well as standard 25g 1-inch needles for regular IM injections

In short — I went through the whole playbook. I was close to giving up on TRT altogether, even though my natural testosterone production was absolutely terrible.

Then something happened that changed everything. After 6 years.

And no — it's not about E2. It's not about injection frequency. Even though those seem to be the answer to 95% of all TRT problems discussed on here.

Like a lot of people, I had been influenced by the modern online TRT meta — switching to insulin pins, going subQ or shallow IM, and treating a standard deep intramuscular injection with a 1-inch needle as the default for regular IM.

Almost by accident, I decided to try switching to **27g 1.5-inch needles** for my intramuscular injections.

And then everything changed.

My brain fog vanished completely. My sense of wellbeing improved. My energy went up. My libido crept back. **Every single side effect I had been battling for years — gone.**

For context: I'm not overweight. I'm a 5'11" (180cm) male, 187 lbs (85kg), been training for many years, and sitting around 15% body fat.

I'm not saying this will make a difference for everyone. But something is clearly happening in my body that causes side effects — particularly brain fog and difficulty concentrating — when I inject in a way that allows the oil to leak into subcutaneous tissue. I simply cannot tolerate it.

And my bloodwork never showed why. My E2 wasn't spiking. Everything looked perfect on paper while I felt awful in real life.

I'm writing this post not to say that subQ or shallow IM doesn't work — for many people it clearly does. But for me, the difference was night and day. The fix was simply that my injections weren't going deep enough into the muscle.

If you've been struggling to feel good on TRT despite dialing in your protocol, this might be worth considering.

**TL;DR:** Tried every TRT protocol and dose combination over 6 years. What finally worked was deep intramuscular injections using 1.5-inch needles into the glute. That's it.
