Hi, bobo questions about the deed of undertaking. I'm planning to go abroad and one of the requirements na need ipass (as per the dost guidelines) is yung deed of undertaking. With this, I have some questions regarding sa fields na need lagyan ng infos:

1. Pwede bang family member yung co-maker?

2. Need ba na may CTC/cedula yung scholar?

3. Yung witness, do they need to be some kind of lawyer? Or anyone would be accepted as a witness?

4. What to put sa dalawang blanks after ng "personally appeared," is it me and the co-maker or the two witnesses?

5. Ano yung ilalagay sa "Witness my hand and seal"
