Hey everyone,
I’m currently trying to put together a feedback form for the musicians and singers at my church, and I want to do this the right way.
The goal is to better understand both the technical side (gear, monitoring, setup, rehearsal flow, etc.) and also the personal experience (how comfortable people feel, communication, what could be improved, etc.).
I don’t want this to turn into just another generic survey that no one cares about — I actually want to gather useful insights and make meaningful improvements for the team.
For those of you who have done something like this:
- What kind of questions have been the most helpful?
- Are there any “must-ask” questions you always include?
- How do you approach sensitive topics like frustration, lack of comfort, or team dynamics?
- Do you prefer anonymous forms or open conversations?
- Any mistakes to avoid when collecting this kind of feedback?
If you’ve got examples of questions or ideas, I’d really appreciate it.
